Little Makos (Pups) Youth Baseball Camp by Midwest Makos LLC — Ages 5–9 • Monday & Wednesday Sessions • May–September
I, the undersigned parent or legal guardian of the minor child named below (the "Participant"), acknowledge that participation in the Little Makos (Pups) youth baseball camp operated by Midwest Makos LLC (the "Organization"), including all practices, drills, scrimmages, and related activities, involves inherent risks that cannot be entirely eliminated regardless of the care taken to avoid them.
These inherent risks include, but are not limited to:
I confirm that my child's participation in the Little Makos (Pups) program is entirely voluntary. I have read and understand the nature of the activities involved. I believe my child is physically fit and capable of participating in the program, and I have not been advised otherwise by a medical professional.
In consideration of the Participant being permitted to take part in the Little Makos (Pups) program, I, on behalf of myself, my child, and our respective heirs, assigns, and personal representatives, do hereby release, discharge, and hold harmless Midwest Makos LLC, its owners, officers, coaches (including Devan Ahart, Nick Plummer, and Duncan Gillett), staff, volunteers, agents, and any facilities or property owners used for program activities (collectively, the "Released Parties") from any and all claims, demands, causes of action, losses, damages, or liabilities of any kind, whether known or unknown, arising from or related to the Participant's involvement in the program.
In the event that the Participant requires emergency medical attention during any program activity and I, the undersigned parent or legal guardian, cannot be reached in a timely manner, I hereby authorize the coaches, staff, and representatives of Midwest Makos LLC to:
I understand that any known allergies, medical conditions, medications, or special health considerations for my child should be disclosed on the registration form in the designated medical information field. It is my responsibility to keep this information current and to notify the coaching staff of any changes.
I grant Midwest Makos LLC and its authorized representatives permission to photograph, video record, and otherwise capture the likeness of my child during their participation in the Little Makos (Pups) program, including practices, games, events, and related activities.
I further grant permission for the Organization to use such photographs, videos, and likenesses for the following purposes:
I waive any right to compensation for such use and acknowledge that images may be used without identifying my child by name unless separate written consent is provided.
The Little Makos (Pups) program is committed to providing a safe, positive, and encouraging environment for all participants. The following expectations apply to both players and their parents or guardians.
All fees are non-refundable once the program begins. By registering, you acknowledge and agree to this policy.
All cancellation requests must be submitted in writing to info@mwmakos.com. The following policies apply based on your selected pricing plan:

Sessions cancelled due to inclement weather, unsafe field conditions, or other circumstances beyond the Organization's control will be rescheduled to an alternate date whenever possible. Weather-related cancellations are not eligible for individual session refunds. The coaching staff will notify registered families of any schedule changes as promptly as possible.
Midwest Makos LLC is committed to protecting the privacy of the children enrolled in our programs and their families. In compliance with the Children's Online Privacy Protection Act (COPPA) and applicable state privacy laws, we adhere to the following data practices:
All personal information collected is used solely for the purposes of program administration, including registration processing, session scheduling and communication, emergency contact procedures, and payment processing. We do not use children's personal data for marketing purposes.
We do not sell, trade, rent, or otherwise distribute your personal information or your child's personal information to any third parties for commercial purposes. Data may only be shared with payment processors as necessary to complete transactions, or as required by law. All personal data is stored securely using industry-standard encryption and access controls.
